  • Risk Ranking Matrix

Hazard

Event that can best-represent the accident scenario
Every hazard would have mutliple causes and multiple consequences.
A consequence is to be chosen as the credible-worse consequence and used for risk ranking.

Recommendation

An action would be either to provide additional safety measure or to perform a detailed analysis for further informationn for risk assessment.
It should be as specific as possible reminding the context in which the action was formulated.
Designating responsible parties will be helpful, which is not always available.

Risk Ranking

'High' risk hazards are not tolerable.
'Medium' risk hazards are to be re-visited for improvement.
'Low' risk hazards can be managed for further risk reduction.

The risk assessment matrix for risk ranking shall reflect the value of the company and the society in which the comapny operates.
